Student Physicals – Tuesday, August 1 2012

Lee Ann Mitchell and Nancy Allen, our resident Nurse Practitioners, will be performing marching band participation physicals Tuesday, July 21st from 5:30pm to 8:30pm in the North Middle School band room. Parents must complete and sign the medical history portion of the form before the student will be given a physical.

The cost is $10, which will be donated back to the band. Parents with financial hardship must speak with Mr. Froedge to request waiver of the fee.

Please remember, all students participating in marching band are required to have a physical.
